try to match you with
the provier of your
choice or the one
that most suits your
needs.
If you are referred
to us by another
physician, we need
copies your medical
records for our
physician to review
prior to your
appointment. Please
ask that they fax
those records to our
office at
208-336-8248 as soon
as the appointment is
made.
We will be sending a
packet of information
in the mail to you
after scheduling your
appointment. Please
complete and return
the enclosed forms
within 2 days. Be
sure that you obtain
any primary care
referral that is
needed by your
insurance
